Contracts:
Include confidentiality, initial assessment, diagnosis and treatment planning, screening for group, payment planning, and safety contract as needed.
FEES:
Cash and check only, and to be paid at the time of service.
A receipt will be provided for tax purpose, and for those who like to get reimbursed from their insurance company.
